The Hounslow GP Consortium CIC and its voluntary sector partners, Groundwork London and H4All CIO, have multiple new posts available for committed health and care staff to work across Hounslow’s 5 Primary Care Networks as:

Team Leader & Health Coach – several posts

Team leaders will provide line management support and supervision to the link worker teams, provide advice and support on the role of a link worker, manage a caseload of patients with a range of support needs focusing on Individual Goal Setting, Motivational Interviewing and Health Coaching. They will also liaise with the various partners and Steering Group on behalf of the Link Workers they manage.
