Description  
Would you like to be part of the exciting community health programme in Hounslow? 
 
The Hounslow GP Consortium CIC and its voluntary sector partner, Groundwork London, are looking for committed health and care staff to work across Hounslow’s Primary Care Networks as Social Prescribing Link Workers (SPLW).
Link Workers provide connections between the GP and community services that can support residents to address their wider health and care needs.
The Link Worker programme is a national initiative from NHS England and NHS Improvement and we are seeking individuals with a background or interest in health, care, public health or voluntary sector services.
